| Description: | A Social Worker is sought to perform the functions of a Child Psychiatric Liaison Coordinator (LC) for innovative state-wide CAP PC (Child and Adolescent Psychiatry for Primary Care) project to provide training/education, consultation, and linkage/referral services to primary care physicians.
The Liaison Coordinator will report directly to the Columbia University/New York State Psychiatric Institute child and adolescent psychiatrist. This is not a research position.
Very occasional travel within New York State may be required. |
| Duties: | The LC will answer the toll-free physician hotline and be the first-line clinical contact for primary care doctors and their staff requesting consultation from the child psychiatrist. In addition, the LC will provide linkage and referral services to primary care physicians requesting guidance for the patients and families in their practices who would benefit from mental health care and treatment.
Other key responsibilities include:
1. Help arrange selected face-to-face evaluations of patients at a maximum of one per week.
2. Develop and maintain comprehensive listing of mental health (private and public) resources for all counties within the site catchment region.
3. Provide referrals to all physicians within our catchment area.
4. Recruit interested physicians to attend trainings.
5. Help organize space, food and presentation requirements for a three day physician training.
6. Organize and keep minutes for every other week case-based conference calls.
7. Contribute to monitoring and completion of all NYS OMH required data reporting.
8. Represent CAP PC at local and state meetings, as determined by site child/adolescent psychiatrist.
9. Establish and maintain cooperative relationships with primary care physicians, and other major stakeholders in the project, including OMH, AAP, AAFP & Families Together.
10. Work with site child/adolescent psychiatrist, and other site Liaison Coordinators to promote the project and referrals.
11. Conduct outreach and informational presentations for families & professionals.

| Qualifications: | Education:
Masters degree in Social Work Or Related Field required
Experience:
Minimum of 3 years experience in clinical work with children and families with mental health needs
Experience collaborating with physicians and other health care providers, social service agents, and school personnel.
Other qualifications:
Knowledge of community child mental health resources, supports and linkages;
Knowledge of Mental health laws and regulations;
Knowledge of school systems and supports available for children and adolescents;
Knowledge of social service system, including child protection services, and supports available to children and adolescents
Preferred skills:
Excellent communication skills (verbal, including by phone, and written, including email, clinical reports) and strong attention to detail;
Strong team player;
Strong computer skills, minimally with email, Word, Excel, Outlook, Power Point, Internet
Strong mental health clinical skills in working with children and families;
Effectively works with others, including physicians and co-workers;
Listens effectively, tolerates differences and promotes diversity.
Bilingual (e.g. Spanish and English) considered a valuable asset, but not required
